The Torturette consists of two parts. One is the stock and the other is the torture device. The trick is that you place your arm into the side of the stock. You then take the torture part, which is a bed of nails, and insert it into the stock. You then remove the nails and your arm is uninjured. They are real nails and you do see them come out the bottom of the stock.
